Edmonton O-day'min Primary Care Network

Type of Organization: Associate

Welcome to the Edmonton O-Day’min Primary Care Network, the second-largest PCN in the Edmonton zone. We take pride in our dedication to transforming primary health care through innovative, people-centred services.

Our comprehensive programs focus on complex care, chronic disease management and prevention, ensuring tailored support for individuals to manage their health effectively, and promote long-term well-being. Recognizing the critical role of mental health and addiction services, we create a compassionate environment, providing non-judgmental support for those in need.

EOPCN is committed to supporting physicians and establishing medical homes that serve as the foundation for excellent patient care. We understand the crucial role healthcare providers play in the community, and we provide the necessary supports to ensure they can focus on delivering the best possible care to their patients.

We operate the Seniors Community Hub out of our location, which is a national award winning program. Our team is made up of doctors, nurses and pharmacists who are specialized in providing senior’s care. We will work with you to set health goals that are meaningful to you and link you to the right supports to help achieve your goals. Because we take a wholistic approach, assessments can be lengthy and sometimes done over more than one session. We bridge the gap between primary care and acute care or continuing care to help seniors stay in community, longer.
